Form 1098-T / Tuition Statement

To access your 1098-T form, go to tra.maximus.com

If you are a returning user, login using your existing login and password.
If you are new to the website, follow the instructions under "First Time Students" and sign up using the last 5 digits of your SSN. Please do not use your COD Student ID Number, as this will cause an error.

Students will only receive a Form 1098-T if:
- They paid for classes themselves
- Received a PELL grant or other scholarship/aid
If a student only received a California College Promise Grant (CCPG), formerly referred to as the "Board of Governors (BOG) Fee Waiver", they will not receive a 1098-T.
- A printout of the student's registration statement may be used as proof of enrollment. This statement can be found in Self-Service by printing your schedule from the Plan and Schedule page.
